* Your passport must be valid for at least 18 months from your intended departure date from Indonesia.

• Curriculum Vitae (CV)
• Legalized Relevant Skills Competency Certificate in English
• Legalized Copy of Bachelor’s Degree Certificate in English
• Position and Job Title
• Recent Color Photo (3x4 size) with Red Background

Clear scan or photograph of Personal and Financial Documentation which includes:
• Personal Bank Statement
• Medical Insurance or a Declaration Letter for Medical Treatment at Your Own Expense
• Domicile Letter (required for the onshore process only)
• Photograph of Your Current Visa (required for the onshore process only)

Clear scan or photograph of Company Registration which includes:
• Company Establishment Deed (Akta Pendirian)
• Establishment Decree (SK Pendirian)
• Latest Amendment Deed (Akta Perubahan Terakhir) (if applicable)
• Latest Change Decree (SK Perubahan Terakhir) (if applicable)
• Corporate Data (from Ministry of Law and Human Rights)

Clear scan or photograph of Business Compliance which includes:
• Business Identification Number (NIB)
• Spatial Suitability Approval (PKKPR) Document
• Standard Certificate (if applicable)

Clear scan or photograph of Company’s Financial and Domicile Documents which includes:
• Company’s Tax Identification Number (NPWP) Card
• Company’s Domicile Letter
• Company’s Bank Statement

Clear scan or photograph of Company Visa Application Details, which includes:
• Company's Email Address linked to the Visa Online Account (if applicable)
• Company's Visa Online Username (if applicable)
• Company's Visa Online Password (if applicable)

Username for Foreign Workers Online System (TKA) (if applicable)

Clear scan or photograph of sponsor's identification documents, including the following:
• Identity Card (KTP)
• Tax Identification Number (NPWP)

Sponsor's contact Details, including the following details:
• Complete Address
• Phone Number
• Email Address

Clear scan or photograph of Indonesian Employment Social Security Agency (BPJS Ketenagakerjaan) Card

Clear scan or photograph of Signed Power of Attorney Letter
